You have two different sizes to choose from, so you can select the one that's most comfortable for you. This ring is anatomically designed to provide restriction in just the right places - the spongy tissues that hold the blood. This works by partially restricting the return of blood. The top, constraining part is very flexible - for safety and comfort. The bottom part is more rigid, allowing it to maintain its shape. This should help with maintaining arousal if that's your thing. What I noticed is a noticeable increase in girth. My girlfriend also noticed and found it extremely pleasant. guys tend to place a lot of emphasis on length, but for your partner's sake, girth is where it's at. I purchased the BULLRING 2X STARTER PACK. The largest (size X) is the largest one they make and it’s WAY TO SMALL. I did not see on the Amazon page and couldn't find on the manufacture's website where it says it must be worn on the shaft only, but you can read it on the item's packaging, i.e., after the you’ve received the package. There is no image of the packaging on the Amazon page. I also couldn't find customer service on the manufacture's website, but I was contacted by them through Amazon and they offered me a full refund. They have informed me there is a live chat option on their website, but I haven't been able to find it. I do believe, however, after my correspondence with them, this is a QUALITY, SOLID SMALL BUSINESS who stands behind their product and holds customer service and satisfaction as a core value. They also informed me the instructions for how to wear the device is on the Amazon page, and I did find it, so bad on me. Great idea, and it helps a little, but not tight enough to work well. The medium size has a much bigger inside diameter than the major brand "average size" round ring I have been using. This ring is very comfortable, and I think they have a great idea here. I know most guys don't want to order a "small", but they need a tighter size for those of us who are "skinny" or "thin".
