It can massage any part of your body, temple , back neck, shoulders, upper back, lower back, your temple etc. Just hold and and circle around or go up and down, sideways. You can use just one knob, two knobs or 4 knobs. It depends on what ball reached that spot that is painful...that is when you are doing it yourself. But if someone is doing it for you it is a whole lot more nicer. Put biofreeze or any pain cream that you get over the counter and let this massager glides into it...So..so relaxing when the heat is activated. That hard knot on your shoulder will go away in no time. My only complain about this is that this is for right handed person. I like that it is cheap, works and available. I have sore upper back muscles form regular workouts. I use hot packs and ice to treat the area and needed a way to reach over the shoulder to massage my shoulders. This works ok. Better with someone else to rub but not always available. Don't drop it on the floor as it is brittle and will break. Don't press too hard, it will snap into what I am sure will be sharp pieces.
