Introduction : Il n'existe à l'heure actuelle pas de score de triage hospitalier spécifique des accidents de la circulation. L'objectif de l'étude était d'expérimenter un nouveau score de gravité des patients traumatisés de la route n'ayant pas bénéficié d'une prise en charge pré-hospitalière médicalisée à leur arrivée aux urgences, et de tester sa validité. Matériels et méthodes : Nous avons réalisé une étude observationnelle analytique, sur une cohorte de patients traumatisés de la route se présentant aux urgences adultes du Centre Hospitalier Universitaire de Poitiers par transport non médicalisé, de septembre 2013 à juin 2014. Notre score a été élaboré à partir d'une revue de la littérature et à l'aide de médecins experts. Il contient 27 items, regroupés en cinq catégories : cinétique de l'accident, critères de gravité hémodynamiques, respiratoires et neurologiques, et critères de gravité liés à l'environnement. Le critère d'évaluation de notre score était la CCMU (Classification Clinique des Malades aux Urgences), les patients non graves étant définis par une CCMU I ou II et les patients graves par une CCMU III ou IV. Résultats : Nous avons inclus 273 patients. Le score était bien corrélé à la gravité, avec un score médian du groupe Non Graves égal à 4 (écart interquartile=4), et un score médian du groupe Graves égal à 9,5 (écart interquartile=7) (p
